If you take thyroid medication (levothyroxine, Synthroid, etc.), the timing of calcium or iron supplements is confusing. Thyroid medication is unusually sensitive to absorption, so taking supplements right alongside it can weaken the effect. It’s not hard — you just have to get the timing right.
✅ Calcium, iron, magnesium block absorption → space at least 4 hours
✅ Biotin distorts thyroid lab tests → stop a few days before testing
Why timing matters
Levothyroxine’s effect wavers with even small changes in how much is absorbed. But calcium, iron, and magnesium bind to it in the gut and block absorption. So even at the same dose, less actually enters your body and thyroid control can wobble. That’s why thyroid medication is usually taken first thing in the morning on an empty stomach with water, pushing food and supplements later. Space calcium and iron by at least 4 hours.
| Space from thyroid med | Note |
|---|---|
| Calcium | At least 4 hours (includes antacids and calcium-containing supplements) |
| Iron | At least 4 hours |
| Magnesium, zinc, other minerals | Space them out |
| Fiber, soy (isoflavones) | Can affect absorption |
| Coffee | Can slightly lower the effect — wait 30–60 min after the pill |
Biotin isn’t a “spacing” issue — it’s a “testing” issue
Biotin (vitamin B7) can make thyroid/hormone blood tests read differently from reality by interfering with the assay. Not knowing this can lead to wrong dose adjustments. Stop biotin about 2 days before testing and tell the lab/doctor you take it.
Your meds + supplements — check in 3 seconds
Pick “thyroid medication” and your supplements for spacing and cautions.
Questions people often ask
Q. Thyroid med and calcium — when do I take each?
Thyroid med on waking, empty stomach, with water; calcium at least 4 hours later (lunch or dinner). If a multivitamin contains calcium, space that too.
Q. Can I take iron with thyroid medication?
Iron also blocks absorption, so space at least 4 hours. If you need iron for anemia, spacing lets you have both.
Q. Does biotin really throw off thyroid numbers?
Yes — biotin interferes with the test so numbers can differ from reality. Stop 2 days before and tell the lab. It doesn’t clash with the drug itself.
